Initially, the series opens to a chaotic drama where a doctor-soldier, Ju Ui, is transferred to an abysmal island where none of the doctors prefer to go. People living on the coast are far from crazy; they are simply unbearable. A health center in Pyeondong is simply a city walk to get free medicines. A doctor like Ju Ui feels trapped in a place like this. Already dwelling in trauma, resulting in him being confined to pills, he doesn't want to be around water bodies. A woman with a sunshine smile pulls him inside the ferry before he drowns himself in the water.
The first impression doesn't go well among the clinic folks, but later, things start to go more absurd for Dr. Do. Residents with weird cases don't want the doctor to diagnose them; instead, they claim they know their bodies more than the doctor can predict. At first, the doctor tries to reason with them, but they are far more stubborn than a child. Mostly, people are old, so they assume that doctors are just money magnets for poor patients, and hence, they start to blame the new doctor.

Each day seems heavy on Doctor Do, yet he tries to at least make it to the year. Nurse Ha Ri, also the same ferry girl who carried the poor doctor, hallucinating about Ha Ri's suicide, becomes a legend for coastal folks and an embarrassing moment for the doctor. She helps with the patients and tries to calm them, but holds a soft spot in their eyes. Constantly being nagged and humiliated by coastal people, the doctor now seems to lose his calm.
As the series progresses, the doctor starts to feel welcomed by the people. Even though tides do turn opposite for Ji Ui, people start growing warmer towards him. Ha Ri and Ji Ui too start to open up their hearts, and wounds start to heal, but still things aren't what we are supposed to think. There is a constant jump from the past to the present that resurfaces back and forth between Ji Ui and Ha Ri. Still trying to live with the pain when Ha Ri's grandmother is diagnosed with breast cancer, and she refuses the treatment.

This hampered Ha Ri's real relationship with Ji Ui, who supported her. While things seem to be going off the rails, Ha Ri understands that the time to be with her grandmother is far more precious than the treatment. But unfortunately, that moment too is short-lived. Ha Ri heals from the loss, but now doctor Do seems to face a similar trauma, and somehow sprouts back. A young patient who almost died keeps the clinic on edge until they hear that she is fine. Also, a patient with acute appendicitis was about to be taken by helicopter for proper diagnosis, witness a crash while Ha Ri and Ji Ui took him.

Once again, Ji Ui is blamed by the family of the pilot, and this time, he locks himself in grief. In the meantime, Ji Ui's ex enters the island to rectify her mistakes, but this also doesn't work. It's revealed that Ji Ui's fellow doctor was fatally injured while swimming. He died while they tried to revive him. Dealing with the past, and now another incident rewinds that trauma. Despite this, Ha Ri refuses to leave his side. She investigates the helicopter incident and proves his innocence. As things start to go smoothly, his medications start to question his integrity as a reliable doctor.

Although circumstances don't seem in Ju Ui Do's favor, it turns out that the village folks show their faith in him. His transfer to the governor's office and back proved his love for his work. All things finally come to an end, and Ju Ui Do can overcome his trauma and once again start to help people with their trauma.
